Qualified Social Worker/Care Manager?required for Adults Review and Assessment Social Work team in South London
Desired skills
- Post qualified social worker experience essential!!!
- Good assessment skills ? ability to complete Community Care
- Assessments of customers and carers
- Using personal budgets to set up self directed support
- Support planning / setting up managed services
- One year post qualifying experience
- Liaison with OT colleagues working in reablement
- Work according to identified timeframes
- Strong literacy, numeracy, communication and IT skills
- Flexible and adaptable
- Knowledge of the NHS and community Care Act and other relevant Legislation
- Car user desired
